Fall Practice Field Application

Your team MUST be registered for the upcoming season prior to submitting this form. All applications received for non-registered teams will be automatically discarded.

* All OGSA Fall League teams are allowed one practice slot per week through the end of the fall. A second practice slot may be available however field availability is based on league registration numbers which is unknown at this time.

* Practice fields before 8/8/2021 are first come first serve. Lights and machines will not be provided.

* Option 1 - Preferred practice time and location
* Option 2 - Alternate practice time and location (if preferred is not available)
